Accessibility evaluation report

WCAG-EM Step 5. Fill this in and print it. Every section below is required by the methodology — the one people leave out is the sample, and it is the one that makes the report checkable.

1. Summary

State it literally. "The 8 pages evaluated on 9 September 2026 did not meet WCAG 2.2 Level AA; 15 criteria failed." Never a percentage.

2. Scope

3. Sample evaluated

List every page, with the reason it was chosen. This is what allows another auditor to reproduce your work.

5. Findings

One row per defect, not per instance. If one component fails on six pages, that is one finding affecting six pages.

8. Limitations of this evaluation

Always include this. A sample can disprove conformance but never prove it, and no audit substitutes for testing with disabled users.
